* Poultry operations housing 2,500 birds or more must register annually.
* Registration is from January 1 to March 31 every year and covers the previous calendar year.
* Registration must be submitted to the Conservation District office (please mail a signed form for each operation with a check made to WCCD)
* There is a $10.00 fee for each registered farm.
* Each registration form must have information regarding:
            Litter produced, applied, or transferred
            Litter analysis including Water Extractable Phosphorus (WEP) (good for 5 years)
            Soil test analyses (every 5 years) 
            Acreage available for application

PLEASE NOTE: If your poultry farm was sold or shut down in any registration year, you must still file a registration.

Short Form: Use this form if you have registered your poultry farm in the past
